This form is intended for multiple boilers (more than 1) that require an Installation Permit. Please complete the spreadsheet “Multiple Boiler & Pressure Vessel Template” and attach it to the application.

IMPORTANT: To avoid processing delays, prior to completing this application, please ensure that the equipment on this application is a BOILER.
Initial Licence to Operate
For new equipment only, once your acceptance inspection has been completed and we have determined that your equipment requires an annual licence, it is now included with your installation permit application and will be sent out with your inspection report. The Initial Licence to Operate is no longer a separate application or fee.
How to Apply

To pay via EFT or Cheque, complete the Installation Permit – Boilers – Payment via EFT or Cheque form.
