Reporting Unsafe Conditions, Incidents and Accidents (February 2022)

Overview

To ensure the safety of regulated Elevating Devices, Amusement Rides and Passenger Ropeways in Saskatchewan, all unsafe conditions, incidents, and accidents must be reported to TSASK’s Chief Inspector.

What is an unsafe condition, incident, or accident?

Unsafe Condition

Something that varies from the normal, accepted safe operating condition of a regulated device which may result in injury, death, or property damage if not corrected. This may include fire, water exposure, impact, vandalism, lightning strike, etc.

Incident

Any incidence with a regulated device that results in an adverse consequence to a person or property.

Accident

Any occurrence with a regulated device that requires medical attention, causes health impairment or results in serious injury, or death.

What to do if there is an unsafe condition, incident, or accident:

1. Immediately remove the device from service.
2. Report the issue to TSASK within 24 hours; both the Owner and Contractor maintaining the device must complete in full the TSASK Report an Incident form
3. Except for making the site safe or rescuing an injured person, do not disturb, alter, remove, or destroy anything related to the unsafe condition, incident, or accident until a TSASK Inspector gives permission to do so.

The regulated device cannot be used until the cause of the issue has been identified, the device is safe to operate, and a TSASK Inspector has given permission to return the device to service. The TSASK Chief Inspector will determine if further investigation is required.
